Hi,

1. これ、来週の健康診断で使うやつですよ。

From context, it seems as if this should mean "This thing (= notice that has been given to employees) is about next week's health checkup."

But can ~で使う mean "about" (in the sense "concerning", "relating to")?

2. ところで、足立さん、この健康診断記録票ですか、わからないところがあるんですけど・・・。

Is ですか a typo for ですが?

3. ブラウン has found it hard to understand the health questionnaire, and says that the difficult kanji are giving him a headache. In the audio for this dialogue they are both laughing, so obviously it is all light-hearted.

ブラウン: ・・・健康診断って、なんだか体に悪いん じゃないかって気がしちゃいますね。
足立: いや、いや。ま、そう言わずに・・・。

I believe Brown is saying "I'm beginning to think that these health checks are bad for your health!", but I am not sure what the reply means. At first I thought it meant "No, you shouldn't say that!", but I now I am doubting this.

4. 大きい病院で、もう一度検査するようにってなってたん です。

That last string of kana is slightly boggling me. I think もう一度検査するようにって (= ように + って, quoting) means "it says I have to go again for an examination" (referring to a letter), but then is なってた = なって (from なる) + いた?

5. 男性の方は上着を着用してください。

Translation given: "Jackets are requested for men."

Is 方 = かた, meaning "person" (respectful)?

1)
~で使う literally means "use at/in~".

2)
Yes.

3)
The last half, something like (健康診断を)やりましょうよ, (健康診断記録票に)書いてくださいよ is omitted.

4) 5)
Yes.

Thanks Toritoribe,

1. So "やつ" must be a document that is used in connection with 健康診断, not simply a notice that 健康診断 will take place, right?

4. What is the subject of なってた (if, indeed, it has a clearly defined subject at all)?

1)
Yes, that's right.

4)
"The comment/notice on the document" or like that. You can think ~ってなってた is almost the same as ~と書いてあった.
