In that case, put your city and state in the first block and the street address and apartment number (it applicable) in the second block.

Form for registering a change of address or a change in one's personal seal, apparently in connection with postal savings accounts. Definitely from before the post-war war orthographic reforms, but there is nothing on the form which would date it specifically. If you were expecting some WWII / military connection, there is none.

I understand the いいところ part but 下っ端 doesn't make sense, since 参謀本部付けの中尉 and 下っ端 mean the same (下っ端 = underling?). They are duplicated.

Actually, you don't understand the いいところ part. He is saying that a lieutenant is just an underling.
It means "think about you seriously to understand you" or along the line there.

it is said to be the best one. So there is the expression 檜舞台に立つ (tatsu is to stand) which means that
someone is in limelight.
有象無象 is people who are not important or useless. But I don't understand when put together does it mean "You useless people who stole the limelight from me!" or that the useless people will be used to create the limelight for the speaker?
