So how can one tell which is which when they hear かぜ in a conversation.

The context gives the answer. If it's confusing, the speaker would use other words or expressions. We use language to convey what we want to say, right?

Ironic that this particular post has a lot that's dependent on context. Both "that" and "my understanding" depends on the context of this conversation. English has it's fair share of things that depend on context.
