I thought it might be : if that guy lives by his appearance then he has something more attractive than me?

There is no "should" in English, and there is no "than" in Japanese.
It doesn't make much sense. 周り is the subject in 周りが心配になる, thus,it means "others worry (him)".

There is no "than me" there. もっと refers to the comparison between 外見利用して生きていく and 外見利用して生きていかない.
You didn't translate そういうのすげぇ嫌がる.

Thanks for your reply. I will reply more in future.
I thought もっと modifies the word after it? Having another go at translation then: If he uses his looks more then he (will) have something else? That does not sound right to me. The そういうのすげぇ嫌がる was "I/He hates that".

I meant "If A, then more B = now is not A, therefore less B."

嫌がる shows that the subject is not the speaker. Check again the difference between 嫌だ and 嫌がる. ("痛い vs 痛がる" might be easier to understand the difference.)
