On a quick read over, these two parts look fine.

Hello Hirashin,

I believe the exam looks pretty much perfect except number 10. It should be "( ) you and Tom good friends". Instead of "( ) Tom and you good friends.

I hope your students do really well on the exam!

Your example "you and Tom" is wrong. Are you a native speaker of English?

Buntaro, why do you think "you and Tom" is wrong?
I would agree with Jinko that it would be more natural to start with "you and X" instead of "X and you", e.g "Do you and your g/f live together?", one because "you" is in a sense the main or more important element to the question as you would be talking to that person, and two because "Do you.." would be a natural collocation whereas "Do Jim/your girlfriend.." would obviously not have the correct S V agreement, but either one would be fine at the end of the day.

Thanks for the help, Jinko, Buntaro, and Michael. There may be a regional difference in English usage.
If my memory serves right, I read that "Tom and I are good friends" is better than "I and Tom are good friends" because putting "I" first sounds impolite. Is that wrong?
