Respectful request for translation please - war artifact

Dear forum experts -

The attached photos are of a cloth tag attached to a sword at the end of the war, I believe, and apparently detail the name and rank of the owner, and other details?

It would be greatly appreciated if a kind person could translate this - I would like to know if this is a genuine war artifact. Thankk you for your help.

I have just discovered this forum, which looks great - I hope to travel to visit Japan for the first time in June this year (Osaka, Kyoto and Tokyo), so hope i can get some interesting information. I love Japanese history and art.
