I think my MyNa would also work at clinics or hospitals, and I have an appt on the 29th, so maybe I'll give that a try. I got this same card a year ago (with the smaller one), and present those when I visit a clinic or hospital. This is almost certainly due to me being on 後期高齢者医療保険, instead of the regular system for you young'uns.
I've used MyNa for my twice-yearly health check-ups and at my dentist's. The screen always presents a series of consent questions. When I couldn't understand all the prompts, the staff told me not to worry and to "click yes."
I am not sure why you still get your paper card, even when on 後期高齢者医療保険. I received my last one in 2024.
