Translation, please.

For my own clarity: do I read this right to left, top to bottom, or right column first, then left?

asking because I like to put the kanji and their literal word by word translation onto my record page like the attached

The right column is one string of text. (One complete "phrase"). The column next to it is another string of text (another separate "phrase").

You misunderstand Majestic-san's post in the thread you are referring to in the word file. Nakanishi is just 中西. 小囗 refers to the next vertical column, which I answered that it's 小林. The square is a mark to indicate "unidentified kanji/kana" here, as he wrote "something" in the thread.

As Majestic-san wrote above, each vertical column shows a phrase(name in this case) as a set, so they must be on the same center line. 一二三君 is OK in this point, but it seems that the font size of the first kanji "一" is smaller than the rest in your file.
