Help i need a filled sample of the MEXT Health certificate form

hello everyone ,

so my doctor don't know english very well
and i need to show him an example of a filled form so he knows how to fill my form

if someone still have a picture of him form i hope you can share it with me
so i can use it as sample

The form is very complicated and has to have your health details. I do not believe someone's copy will help you. There is a Japanese embassy in your country. I would contact them and see if they can give you the form printed in Arabic. I think someone there would help you with a translation for your doctor. If you Google about the embassy , it gives the address & phone number. Good luck !
