Hi all,
In a book I'm reading if a 座敷わらしい has to leave your house it could bring bad fortune.
So the character says

どうかでていかないでおくれ.....

Which I believe is "please don't leave"

Is it normal to have the honourable お before くれる ？

Also is どうか very casual?

よろしくお願いします

It's not before くれる. It's before くれ

どうか is not casual. (The お before the くれ should provide a clue that polite speech is being used here).

So ください is more polite. Next おくれ, followed by くれ is it then the imperative?

Where does どうか make it fit into the politeness scale. Is it more like pleading as in the English "PLEASE " don't leave. And they are softening it with the お？

That's more likely entreaty than imperative.
Yes for どうか as please.
