News Ambulance charges for non-emergency cases in Ibaraki spark public debate

It's not entirely unreasonable, is it?

Ibaraki Prefecture made headlines last year when it became the first in Japan to introduce charges for ambulance rides in cases deemed "non-emergency," with doctors responsible for deciding which cases qualify. Traditionally, ambulance services in Japan are operated by local fire departments and provided entirely free of charge.

Supporters of the new system argue that a growing number of people have been calling ambulances for minor injuries or mild illnesses, placing strain on emergency services and making it harder for those in genuine need to receive timely care. A prefectural official explained that the policy was introduced out of concern that "lives that could otherwise be saved might be lost" if the situation continued. The pressure on emergency services has been mounting, with ambulances in Ibaraki dispatched more than 140,000 times in 2023 — the highest number ever recorded in the prefecture.

In recent years, more than 60 percent of patients summoning ambulances were taken to 22 large medical facilities in the prefecture with teams capable of providing emergency care. But nearly half of cases were considered "mild" by doctors, according to the prefecture's medical policy section. One patient called an ambulance after "blood oozed from a cut in the fingertip on the right hand." Another patient who had a "cold for three days" summoned an ambulance at the "insistence of a family member." The prefecture's new rule seems to have curbed the number of ambulance rides. Ambulances were dispatched 35,491 times between June and August, down 8.3 percent from the same period in 2024. The prefecture also saw a 19-percent decline in ambulance patients who did not require admittance to hospitals.

Doctors worry that charging for ambulance rides in Ibaraki might make some people afraid to call for help, even in real emergencies. They warn that this could be especially dangerous for people living in remote areas where ambulances are the only way to reach a hospital. Fees range from 1,100 to 13,200 yen, but there are growing doubts about whether hospitals are judging which cases count as non-emergency cases fairly. The prefecture has listed twelve symptoms that shouldn't be treated as urgent, yet how those rules are applied is being widely debated.

Maybe a base rate for any ambulance ride, eg, ¥3000-¥5000. (Then a surcharge for anything egregious?)

Why a base rate if you have a 'free at the point of service' health system? It should be free for life-changing medical emergencies.

It makes perfect sense. The number of people who call out ambulances, attend the Accident & Emergency or even go to their general practitioner for minor sniffles, coughs (of <10 days duration) and muscle pains after exercising etc is astonishing. They clog up the system and slow the care for genuine emergencies. 'Accident & Emergency'…the clue is in the title! Deep down they're know they're doing it and being charged for their desire might make them think twice about using the facility incorrectly.
