What would be 請求先情報 equivalent for US bank?

I tried purchasing something for the first time on Japanese Amazon with a US debit card, and it said that my account is locked and I would need to send a picture of a 請求先情報 which should include the last four digits of my debit card or my bank account number, my full name and address and phone number. What is this document called in US?

The only thing that would have that information is a billing statement either from the credit card company or the issuing bank. But if this is one of those prepaid cards you can get at a convenience store, I don't know how you would get this. I would call the help line in this case.
 
I just checked my online statement and it shows last four digits of account number, my name and address, but not my phone number. (but I just noticed that my phone number isn't necessary)

I'm getting this message by email (declined), when I sent a PDF file of my monthly online payment. But they don't seem to explain why.

You're correct, there is no specific details here. It says the account/transaction is frozen until you can confirm your billing information but it doesn't say, what, specifically is missing. It says to sign into your account and follow the instructions to confirm your payment. Maybe there are more instructions there?
Also "when uploading documents your name, address, payment method and related transaction information should be clearly displayed." It also says for security reasons to only upload the last four digits of the credit card # that you are trying to use. Maybe that's what's missing? Did the info you send include the card # so they can match the info you sent to the card you used in the transaction?

Would that mean if I didn't spend any money and hence didn't have any transaction for a particular month I couldn't use the monthly statement for that particular month?

But does it show something that relates to the number on the credit card (which is probably not your account number)?
I think they are talking about the transaction you attempting now. Nothing to do with your historical transactions.

But how could they associate your statement and account number with the credit card that you tried to use? I think that may be the issue.
 
What I did was since my credit card number was not on the monthly statement, I captured the webpage that had both the last four digits of my credit card and last four digits of monthly statement displayed.

This time they sent an email saying they'll just close my account, without explaining what was lacking.
