Actually the words 應召 imply that the person on the flag is being drafted, so this indicates it is a wartime item.
However there is no guarantee that the item is not a fake.
(It seems OK to me though, and it might even be possible to locate this person's family).
Niigata College of Medicine, Surgical Department would be the presenter of the flag. It still exists as Niigata University, Faculty of Medicine, so you might be able to contact the recipient's family, as Majestic-san wrote.
