I'm just realizing... How do you say:

Thank you for __(verb phrase)___...

So, if some one took care of you.. "せわをする"
How do you say, "Thank you for taking care of me."

Also, How do you say, "I'm sorry for ___(verb phrase)___"

Example: If I smoked and wasn't suppose to... using the verb "すう"
How do I say. "I'm sorry for smoking!"

I just have no clue about how to connect the phrases with the "thank you" or "sorry"

(大変)お世話になりました。(Simplest way to say the former).
せわをする = (Without context, you take care/will take care of someone/something).

ご迷惑をかけてすみません。(I'm sorry to trouble you). Try to figure out another example sentence
...

Ok.. Seems like ~て form + すみません for the second one.. The first one was a little too specific... I want something more general so I can use it for "Thank you for picking me up at the airport" or "Thank you for paying for my meal..." or anything....I just want to know how to connect "Thank you" and a verb that was done in your favor.. >.< Thanks for the reply though...

It's the same, well depending on the status of the receiver. ~て + moratte + arigatou gozaimasu etc. 😅
 
wouldn't ○○してもらってありがとう。。。 be thanking them for soliciting the ○○ action (presumably from someone else)? Wouldn't just ~てありがとう or ~てくれてありがとう suffice?

For what the OP wanted, I think of もらって as more polite but often interchangeable with くれて in the sense of a giving and receiving meaning to express an act where an agent does something for the benefit of somebody else. Of course there are a few advanced applications where one or the other is not appropriate...such as you mentioned one usage of もらって referring "to get" in the sense of asking or something or influencing the speaker in some way. It would be easier with a context. :)
 
もらう is the expression on the point of the view of the receiver, so you can use this with a verb which expresses the receiver's emotion; うれしい, ありがたい, 感激する, etc.. On the other hand, ありがとう is mostly used when to say directly to the giver of the favor, that's why くれる/下さる; the verb that the giver is the subject is often used with.

彼におごってもらってありがたかった(expressing the speaker's emothion)
彼がおごってくれてありがたかった(expressing the speaker's emothion)
おごってくれてありがとう(gratitude word to the addressee = the giver of the favor)

Oh, thanks once more Tori ! 🙏 Forgetting 下さる was an honest mistake. But now it appears either 下さる or いただいて are the better options. They are compatible with ありがとうございます. Whereas くれて/もらって don't work on the formal levels. That may be the most urgent problem right now. :)

゛ありがとうございます゜が敬語・丁寧語だけど, 前文で敬語を使わないからちょっと違和感があると思います。

していただいてありがとうございます は問題ありませんね。
 
前のポストのちょっとわかりにくそうな点を書き換えて おきました。😅
「下さる」「頂く」「ありがとうございます」はどれも 高位の敬語なので、-pre masu form と使われることも多いですね。「お送り頂きありがとう ございました」みたいな感じで。

は正しいのですが、
~していただいてありがとうございます
は間違いです。(日本人でもよく間違って使っていますが。)

~してくださる は相手が自分にたいして何かしてくれたという意味ですが、
~していただく は自分が相手になにかしてもらったという意味です。
「ありがとう」は相手の行動に対しての感謝なので、「いただく」=自分がもらった、つまり
自分自身に対しての感謝になってしまいます。

~てくださる、~ていただく、という形だと混乱してしまいますが、
~をくださる(give)、~をいただく(receive)に置き換えてみるとよくわかると思います。

先生が(私に)本をくださった。
(私が)先生から/先生に本をいただいた。
先生にお礼を言うときには
「本をくださってありがとうございます」になります。

違和感は恐らく動作主、敬意の送り手・受け手の問題ではなく、敬意の程度の問題から来ています。指針中の例文中にある補助動詞としての使い方だけでなく、主動詞としても同様に。

下に行くほど違和感が減る感じがしませんか?(僕だけかもしれませんが。😅

「もらう」には敬意がない、「下さる」「頂く」では逆に敬意が強すぎてそぐわない、という理由で単独の「ありがとう」に対しては使いづらいというのもあるんでしょうね。
ただ、こういうややこしい問題を避けるため、「本(を)ありがとう」「先日はご本(を)ありがとうございました」のような略式表現も会話ではよく使われますね。:)
