Quick question; How are you?

Hello I am taking a Japanese course in school and I had a quick question. How would I say to my teachers "how are you?".

Also on a side note if I were to ask him where he lived would this be correct? Sensei wa doko ni sunde imasu ka?

Yes. Another (more polite) version is "Sensei wa doko ni osumai desu ka?"
