Kanji on old Japanese plaster mask

Hello, I was wondering if someone could translate these characters on the back of an old Japanese plaster mask. Thanks!

The only one I can read is the center vertical line
建御名方命 - Takeminakata-no-mikoto
It's the name of a mythological god of Japan.

The far left line would be the name of the artisan who made the mask. Ends in 庵, but that's all I can get.
