Certificate oh health scholarship

I'm applying for the MEXT scholarship to study in Japan and the embassy is about to send my application to Tokyo but I need to give them the certificate of health. I've been to see my GP and he doesn't really know how to do and i don't have much time left. He has filled what he could but now I need the x-ray and blood test. Does anyone know how i could get this done as soon as possible?

I'm not sure how it works in the UK, but when I got it filled out my doctor just filled in the first parts, referred me to a lab to get blood taken and a chest x-ray done and then filled in the rest when he got the results back from the lab.
