Sword Provenance - Hidden Items

Hello Everyone - I'm looking for some help regarding a few items on a wooden ticket that came along with a sword that was acquired by my father in law from Japan. he served in the army of occupation in WWII, and bought a sword while he was there. It came with some wooden provenance, and I really need some outside help to figure out what it all means. Any help you can provide would be really helpful. Thanks!
