Ofuda Identification

This is one of the ofuca that came with the senninbari mentioned in an earlier post.
I was wondering if someone might be able to identify the temple it represents?
Attached is the clearest picture I could take.

It's too blurry to read. It would be the name of the Buddha, not the temple, though.

Thank you Toritoribe. I've learned the ofudo is to
Jizo-son. The caption reads, "Scapegoat Ji-so son"

Did you get the original Japanese phrase? I agree with Jizōson since the last three kanji would be indeed 地蔵尊, but "Scapegoat" doesn't seem correct to me.

EDIT:
"Scapegoat" might be the translation of 身代. So, it's possible.

EDIT2:
The second kanji looks like 通. If so, the first two kanji might be the location name (maybe the name of a street).
