Is there always an 窶堋ｨ at the beginning of the sentence when asking for specific information? For example:

おげんきですか?
お名前は?

Also, would you consider these questions polite or impolite?

Not always, by adding the honorific "お" it makes certain words politer, this is not just for questions. Also お is usually for words of Japanese origin, theres also the honorific add on "ご" which is usually for words of Chinese origin. When/which to tag them on is based on the individual words, you just have to know if it takes it. However there are some words which almost always have an honorific to it for example; おなか = stomache, おかし = sweets, ごはん = rice etc...

In addition to Jericho-san's explanation, the honorific prefix お/ご often means あなたの, the possessive case of the second person pronoun, in context as in the OP's examples.
e.g.
ここにご住所をご記入ください。
Please write down your address here.
