If anybody could help me, it would be greatly appreciated

I just received this letter back from Asaminami-ku Ward Office and I was wondering if anybody could translate it for me? I am an amateur genealogist and I am trying to obtain my great-great-great grandfather's koseki/joseki. I got this letter back after I wrote to them asking if they still had his koseki avaliable.

今回、お問い合わせの件（沖田ハヤ様の母の氏名）につ いて回答します。
沖田元平様戸主の除籍のハヤ様のらんには父母名の記載 はありませんが、他のご家族の方のらんを参考にされる ようでしたら、除籍の請求をしてください。
請求をされるる際は、以下の物を揃えて送付してくださ い。

１．申請書
２．申請書の身分証明書のコピー
３．切手を貼り、住所、氏名をｘ入した返信用封筒
４．手数料７５０円

Here's my attempt at a translation. Man it's hard to read handwritten Japanese:

This note is in response to your inquiry of the mother of Okita Haya-sama.
On the joseki under 沖田元平 (it's hard to know how the first name is pronounced, could be Gempei), there is no entry for Haya-sama's parents, but if it becomes apparent that they are referenced in another family's register, please request those records.

In the event that you make such a request, please assemble the following items and send them in:

1. Written Application
2. A copy of ID papers for the application (?? not sure about this, maybe a copy of the ID of the person filling out the application)
3. A self-addressed, stamped envelope
4. A fee of 750 yen

Alas, charges are often applied for such requests, thank you very much for your cooperation. (I was a bit liberal with this sentence; it's kind of just a Japanese way of wrapping up correspondence)
