I need help with translating a site to registrate. /jFlyff

Hey everyone!
I know nothing about Japanese, and I really want to sign up for a game named jFlyff. So I tried to fill in and realized I've no idea what to fill in where! And I cannot copy and paste it to a translate site, so I cannot check what it means.
The site name is flyff.gpotato.jp and I just need help to translate the creating account place. I've no idea whatever it's in Kanji or something else. Anyone is willing to show off their skills to help me?
