Help translating old document

Hi all.

I found this old document inside of an old military paybook from the 2nd Sino-Japanese war.
His name was 伸藤久依 Hisai Shindo. 輜重兵.
He took part in several battles such as Xuixian-Zaoyang(襄東会戦), Winter ofensive (冬期攻勢()作戦), Central Hubei (漢水作戦), South Henan (豫南作戦), and so on....

Could somebody help me with this document?.

I would be very grateful.

Name is probably 伊藤久作 (Itō Kyūsaku).
The address in the middle picture is Sukenobu, Hamamatsu City, Shizuoka Prefecture.

Just an army identity document, stating his name, address, birthdate.
