I'd pick three because it's more natural to describe people as しんせつ than to be しんせつ to flowers.
 
It's describing mother's actions towards the flowers. While it could be plausible that she is kind to the flowers, the question asks what is the best among the provided options, and "doctor being kind/thoughtful to patients" would definitely be a better choice than "mom being kind to flowers".
 
I thought the mother was just in the garden's flowers. How would you just write that so she is just in the flowers in the garden? What is good word for kind to plants?

If I understand your question correctly, it would just be 母は庭にいます。(Mother is in the garden).
In the flowers in the garden... as in "she is in the area of the garden where there are flowers" it would be something like
母は庭の花のところにいます。

しんせつ also works, but given the limited info in the original question, I'd say that among the answers provided, the one where the doctor is nice to patients is the most natural answer.

I don't think so. This says she's kind to the flower place. To say that she's kind when there in the garden you could change to:
母は庭の花のところにいるとしんせつです
 
Or, 母は庭の花ところにいるとき、やさしい気持ちになります。

Or 母は花に囲まれると親切に(or やさしく)なります
