Help translating old documents

Hi all.

I found these old documents inside of an old military paybook from russo-japanese war. I am not sure, but one of this could be somethin related of an injury caused by a gun?
Could somebody help me with this?.

The one with blue printed text is a discharge order from Ōsaka Auxilliary Hospital in Tennōji, for a Mr. Chōsaburō Kitada, dated January 12th, 1905.

The handwritten note is a copy of an order granting Mr. Kitada (Infantry, Private First Class) , the Order of 8th Class. It notes that he was born in November, Meiji 10 (1877), and that he received a wound at the Battle of Nanzan when a bullet peired a joint in his right leg, thereby making him eligible for the above award. This is dated June 1st, Shōwa 6 (1931)

Now I understand the paybook a little bit more, as the 4th division took part in the battle of Nanshan on May 1904.

On the other hand, could I ask you one last favour?
On the last page of the paybook there is a reference to a place. I think this is what is written:

清国孫家阻子

Could you identify this place?.

I think its 咀子, not 阻子. I don't know where it is.

That last page (before the red writing) talks about how he was put on board a ship in the port of Keelung on December 2nd, 1899, and then arrived at Usa port (I think in Kōchi, Japan, but it could also refer to Usa in Nagasaki...two port cities with the same spelling). on the 8th, when he was then transferred to a hospital whose name I can't make out. It goes on to say that he was completely healed by January 30th of 1900, and was returned to the 38th infantry on February 1st, where he remained until November 30th, when his term of active service was completed.

This obviously predates the Japanese-Russian war, so he must have been called back to duty for that war.
