I tried to send a letter to someone and this came up could someone translate it?

お電話番号電話番号形式(99-9999-9999)で入力してください。
会員番号必ず入力してください。

it means "your anal plug and dildo (serial number 99-9999-9999) will be honorifically 1 week late, sorry about that"

Please enter the telephone number(s) in the telephone number style (99-9999-9999). Please ensure you enter everybody's number.

Hmm, I assume you were sending a letter by email to someone with a mobile phone and it stuffed up somehow.
