I don't remember ever having heard 2 and 3 uttered or written. What about you?
 
I can think of times where 1 and 3 would be used, but 2 just isn't used in everyday conversation. If I was telling someone how my day was for instance, and they responded with "I'm glad to be hearing that." I'd probably pause for a second to wonder why they didn't just say "I'm glad to hear that.". An example of number 3 could be if you were coming out of a concert of your favorite band or something, and you really liked it you could say to your friend, "Wow, I'm glad to have heard that."
 
I have heard #3 before but with a little twist. "I'm glad To have heard ..."
But the next two are almost never used
 
"I'm glad to hear that" sounds like someone is glad about what someone else has just said. But I agree with Geostigma that if someone said "I'm glad to be hearing that" while you're saying something, it would make you pause for a second. Another way to say it is "I'm glad to hear you say that" or "I'm glad you said that". Of course, if it's something other than hearing what someone is saying, like maybe music or a lecture, it would be said another way like "I'm glad I heard that" or "I'm glad I listened to that."

To me number 1 is more correct than the others, it is rare to hear number three, but it does still exist out there somewhere.
