In #3, just using 'which' is okay, but 'which one' would be my preference.

Two possible answers in #4 and #19, need a quote mark in #16.

I left #18 in, since it illustrates a (perhaps minor) test design point, along with #3. I'd prefer if one answer didn't stand out in some way. In #18, only one answer has two words, the other answers are single words; and if you accept my change in #3, it'll be that same situation.

The following sentences with "which" are from "Practical English Usage", a popular grammar book published by a British publisher.
What do you think?

>'Which is your husband?' 'The one in jeans.'

>Which and what are often both possible, with little difference of meaning.
> Which/What is the hottest city in the world?

There might be a difference between the U.S. and the U.K.

Two possible answers in #4 and #19
How about this?
④ "(   ) are you going to play tennis tomorrow?" "At school"
(ア) What (イ) Where (ウ) When (エ) How

⑲ When (   ) New Zealand?
(ア) did you visit (イ) do you visited (ウ) have you visit (エ) have you visited
I'm sorry, but I don't get it.
Do you mean you can say "Have you visited New Zealand"?

In #18, only one answer has two words, the other answers are single words;
I guess it will be all right because in #17 there is also a two-word choice but it is not the answer.

The second answer is clearly the best answer, but the first answer is not completely wrong, so it is best to avoid that choice, I think, and replace it with a different answer which is more clearly wrong. It is natural for a person to ask "What bike?" if someone mentions a bike as if they think it is within sight, but the listener cannot see any bike at all. So, for example, if person A and person B are walking, and person A points in a certain direction and says "That bike is mine", but person B cannot see any bike at all, person B might reply, naturally, "What bike is yours?"

As an alternate option, you can just keep the question as you originally wrote it. It is good enough, and a fair test question.

⑲ is better than before, and only one answer is clearly correct, so it is now a fair question. However, I think many students will find it to be difficult and have trouble with it unless you have already clearly taught the usage of those expressions.

Yes. I taught the usage and made the students do its exercises.
