To "Name" From "Name"...

Simple question I guess.

In Japanese, how would you write "To (name here)", "From (name here)"
As in the sense that it is a gift for someone, from another on the front of a package, gift, or card. :mail:

My guess would be something like 名前に 名前から。。。 Of course that is only a guess, and I'm not one for flimsy guessing on things that are important

But you should add a courtesy title as like "-san" or "-sama".
