Short Translation/Confirmation needed

Hi all.
I am hoping someone can help me with a double checking of a translation. I was recently diagnosed with an autoimmune disease and due to how it has affected me, I want to design my first (and likely only) tattoo incorporating the translation.
The disease is Hashimoto's Thyroiditis. I found this online as a translation:
橋本病

Making sure this is the correct translation is the first step I need to accomplish.

I believe this is correct

橋(Bridge) uses the kun reading はし(hashi)

本(Book/Root/Truth/This/Cylinder uses the on reading もと(moto)

And 病(To fall ill is my best translation( uses the on reading びょう(Byou)

Not 100% sure though
 
It's correct. 橋本病 is in EDICT as Hashimoto's Thyroiditis as well as being noted with the English translation in the japanese wikipedia page for chronic thyroid disease.

橋本 is a person's name (Hashimoto) and doesn't really break down into meanings, being a proper name. 病 (byou) as a suffix means 'disease' and is similar to the '-itis' suffix in English.
