Question About Married Names

When a Japanese woman marries, does she take on her husband's last name completely, or is it common to use both her maiden name in conjunction with her husband's last name? Would the answer to my question be any different if the husband was not Japanese also?

In Japan, usually the Japanese use only one family name and never connect two family names. Because family name is very important in Japan.
When a Japanese woman marries, she take on her husband's family name. But if she get adoptive married, her husband take on her family name and he lose his old family name.
But if a Japanese woman marries with foreigner and acquire nationality of her husband, she will obedient to her husband's country custom.
