How to say your allergic to certain foods at a restaurant

My mom wil soon be travelling to Japan, but she has recently come to be allergic for soy (except fermented), stone fruit and hazelnuts. Could you help her out by writing a little text she could show when going to a restaurant?

The text could be:
大豆、核果類とヘーゼルナッツのアレルギーがあります。

核果 is not commonly used in Japanese, and the OP said "soy (except fermented)", so I would write;
発酵食品以外の大豆、桃やさくらんぼ、スモモ、マンゴー等、真ん中に硬い種が入っている核果と呼ばれるフルーツ類、及びヘーゼルナッツのアレルギーがあります。
Well, it's a bit long, though...

You are right, that is much clearer. I suppose the length shouldn't be a problem if she's just going to print it out and show people.

You are right, that is much clearer. I suppose the length shouldn't be a problem if she's just going to print it out and show people.
The length doesn't really matter indeed.
