Help with translation

Hi all.
I have bought a paybook ans inside it there is an old document. I have been trying to translate it, but I have not been able to understand too much.
This paybook belonged to a soldier who fought in Burna. 上等兵 Mr. 廣吉.

Here is the document:

It seems to be signed by 陸軍航技中尉福井治人 Lt. Col. Naoto Fukui (air force). 1942/01/07.
It seems to me that the document talks about this superior soldier(or specialist) performed duty in Saigon and got injured 受傷ス.

If anyone could provide additional information, it will be welcome.

It's a certificate of validation, validating the soldier's (Private First Class, Itō Hirokichi) injury. The soldier was injured on January 7th, 1942, near the docks in the port of Saigon. The soldier sprained his leg around 3:00pm while unloading drum cans that had come from a rubber plantation. The person who wrote and signed the document is Army Logistics Lt. Fukui Naoto (could also be read as Haruto).
