A notice from police?

Hi everyone.
Today we found a notice from the police(?) in our mailbox.
Unfortunately our Japanese is still very poor.
We tried to translate it with google translator. But because it is a notice from the police, we would like to be sure that we understand correctly.
Could you please be so kind as to help?

The police request you to submit information on who is living in your apartment, for cases of emergency. I believe this is strictly voluntary.

We wonder why of all things they left this notice in our flat? Or maybe they did it with all flats here in the neighboourhood?!

It freaked out my mom when they dropped by to do this when she was visiting.

Go to your local koban, fill in the paper, and be done. If there's a fire in your building you want them to come save you.

My koban offered me the form in English, but I declined and used the Japanese form. I would imagine yours will have the English form if you want.
