Hi all. Regarding the phrase in the heading, is this interpreted "Does Ken have a girlfriend?" or is it saying "Is that (person) Kens girlfriend?", or can it be both?

Does Ken have a girlfriend is correct. It can't be both.
To say is that Ken's girlfriend you could say something like
そちらはけんさんの彼女ですか。
