Is 来られた above a passive form with adversity?
I'm not sure here who is adversely affected.
Is it the host or the visitor?
 
That's honorific. Haven't you learned that the passive form of verbs also works as a respectful form?

Only the host can be the recipient of adversity when 来られた is the passive of adversity.
e.g.
来てほしくない奴に来られて困った。

Does passive form used as respectful form apply to all verbs, transitive and intransitive included?
 
Yes. Passive form is commonly used in daily conversation where honorific forms like "お -masu stem になる " or special honorific verbs like いらっしゃる or 召し上がる sound too polite, i.e., distant/remote/standoffish.
