Looks very good.
⑤( ) your brothers high school teacher? (ア) Is (イ) Are (ウ) Do (エ) Does
This one doesn't seem right.
Is that your brother's high school teacher? would sound good.

⑱ I went ( ) the nearby department store.
(ア) shop (イ) shopping (ウ) to shopping (エ) for shopping
I think you're missing a preposition.
⑱ I went ( ) at the nearby department store.

Again, looks good, and in addition

㉔ I always go to school ( ) eight o'clock. (ア) in (イ) on (ウ) with (エ) by

Something sounds strange with this sentence. You can say "get (somewhere) by...." or "leave (for somewhere) by ....." but not "go ..... by ...."
If you say you have gone to school by 8, does that mean you have left home by 8 or arrived at school by 8? I wouldn't know.
I think it's because go is about the whole process of moving; the travelling itself. It doesn't literally mean the point of departure although that is what we usually understand it to mean as you can't arrive somewhere immediately. It's funny but I go to school at 8 would be fine but I go to school by 8 would not.

For ㉔, what @Michael2 wrote is correct, but for your students, I think it is simplest to revise the problem as follows:

I always come to school ( ) eight o'clock. (ア) in (イ) on (ウ) with (エ) by

Or keep the same format but change the example such as
I always go to bed ( ) midnight. (ア) in (イ) on (ウ) with (エ) by

Thank you for your ongoing help, mdchachi, Michael2, and joadbres.
For 2-㉔, I'll adopt joadbres' idea. It's understandable for my students.
For 2-⑤, how about this one?
( ) all your brothers high school teachers? (ア) Is (イ) Are (ウ) Do (エ) Does
It's funny but I go to school at 8 would be fine but I go to school by 8 would not.
Then what does "I go to school at 8" mean? Is it different from "I get to school at 8" or "I leave for school at 8"?

I go to school at 8 is ambiguous. It can mean either you leave at 8 or you arrive at 8. So you would need to figure it out from the context.
