Meiji era document, please help

Hello!

I have this old document in my collection, I think is a Meiji era (maybe Russo-Japanese war), could someone translate it? (no neccesary a perfect translation, just to know date and what is wrote in the doc), thank you!!

It is a certificate of the awarding of this medal, 7th grade (lowest):

金鵄勲章 - Wikipedia

Awarded to a Sergeant of artillery in the Imperial Japanese Army, April 1, 1904 for service in the Russo-Japanese war. During this war the medal was awarded to 109,600 men. The certificate also indicates he was to receive an annual pension of 100 yen.

His family name is Yamashita. I'll let somebody else tell you the first name. I'm terrible at reading first names.
 
YAMASHITA Tohsaburo, I would guess.

I think it was awarded in 1906, for services rendered in 1904~1905.
