Making a reservation in Japanese

Hey everyone! So I'm going to Japan soon and I'll be going to a few smaller towns in rural Chûgoku, one of which I'll be staying the night.
I'm trying to fill make a reservation, but I have to write it in Japanese because the owners of the hostel won't understand English, and I'd like a bit of help!
I'd like to start off by apologising for my lack of Japanese skills, I think something like '私はあまり日本語が話せません,ご迷惑をおかけてしてすみません' would work?
Then I'll just add the details of my reservation.
'ゲストハウス古民家en' の 予約をお願いしたいんですが。
チェックイン: 6月25日
チェックアウト: 6月26日
和室 女性(相部屋)は は大丈夫です。
Is '食事なし お願いします' or '1泊 素泊まり' okay? I'm trying to say 'no meals included'.

I think this is more or less what I will write (+ email address, nationality and my name in their already assigned boxes).
It's important that I do this right so there are no misunderstandings and I'm not left without a place to stay, so I appreciate your help. Thank you!

BTW, under the e-mail box it says '半角英数字で入力して下さい'. What does this mean? Translator isn't much help.
 
Yes, your translations work fine. 食事はなしでお願いします and 素泊まりでお願いします are both OK.
半角 refers to half-width characters, i.e., usual alphabet and numbers.
