I need help with what to say to my doctors

Hi guys,
so this is my first post here. I need your help. I'm currently living in Kyoto and I have an ingrown toenail. I've been to a doctor today who claimed to speak English but didn't really which is not uncommon around here, and he just kept repeating that I should switch bandages daily which I have been doing for about 4 month already. So for my appointment on monday I thought I'd prepare something to say, since my Japanese is really basic.

So I basically need help saying this:
I've had this infection for 4 month, and I have them very often. I'm not in pain, it just won't heal on it's own. I have been to surgeon in my homecountry who already wanted to give me surgery, but that didn't work out since I left for Japan. I think the only way to get rid of it is to get surgery on my toe.

4か月前からこの感染症は続いていますが、今までにもしょっちゅう罹っています。
痛くはないのですが、自然には治らないんです。
母国でかかっていた外科医の先生は手術をしたがっていたのですが、日本に来ることになったため実現しませんでした。
完治させるには手術する以外にないと思うのですが。
