Help with WWII item from grandfather

My grandfather was an army captain during WWII. He was a medic, but did some fighting in the jungles of Burma and India under Merrill and Stillwell. My father tried to get some help from a Japanese coworker, but he said what it said was so horrible that he didn't want to tell my dad what it was. Any help would be very much appreciated. Thank you.

The picture of the flag is reversed.
The flag was presented to 椎木辰夫 Shiiki (surname) Tatsuo (male given name) by 後藤寺町立東青年学校 Gotōji Town Higashi Youth School (a kind of vocational school) located in the present Tagawa City, Fukuoka Prefecture.
